Your clinical health information — medical records, diagnoses, treatment notes, and billing — is governed by our HIPAA Notice of Privacy Practices, which you receive and sign at your first appointment. Appointment request forms — name, phone number, email address, preferred location, and preferred appointment time. Used solely to contact you and schedule your visit.
Contact form submissions — name, email address, phone number, and your message. Used only to respond to your inquiry.

Any health details you voluntarily include in a message or appointment request are used only to route your inquiry to the right care team. This information is not entered into your medical record until you become a patient and complete your clinical intake in person.
